Version 2026-10-21
The company that sent you the documents sends you the documents on this page through CRM For Claims, an online system for preparing and signing documents. Federal law (the E-SIGN Act) and Illinois law (the Uniform Electronic Transactions Act) let you receive and sign documents electronically if you agree to it. Please read this before you click START.
If you agree, the company that sent you the documents will give you the documents in this signing request, the signed copies of those documents, and the notices and disclosures that come with them in electronic form, and you will sign them electronically.
You can ask the company that sent you the documents for a paper copy of any of these documents at any time, before or after you sign. The paper copy is mailed to you free of charge. To ask for one, use the contact details below.
You do not have to sign electronically. If you prefer paper, do not click START: contact the company that sent you the documents and ask to sign on paper. You can also withdraw your consent later by telling the company that sent you the documents through the contact details below. Withdrawing does not undo documents you have already signed electronically. After you withdraw, you receive paper documents, which can take longer.
Use the same contact details to tell the company that sent you the documents about a new email address or phone number, so that copies and notices reach you.
If these requirements change in a way that could stop you from opening or keeping your documents, you will be told about the change and asked to agree again.
When you finish, you can download the signed documents on this page. If the signing link came by email, the signed documents are also emailed to you. If it came by text message, the text you receive after signing links back to this page, where you can download them for 7 days. You can always ask the company that sent you the documents for another copy.
When you click a Signature or Initials box, your name is placed there in a signature style. When you click FINISH, you sign all the documents in this request. You agree that this is your electronic signature and that it has the same legal effect as signing the documents by hand.
To show who signed and when, the system records the date and time of each step, the IP address and the browser you used, and a SHA-256 fingerprint of each signed file. A Certificate of Completion with these details is kept with your documents.
The signing page shows this disclosure with the name, phone number and email address of the company that sent you the documents. To ask for paper copies or to withdraw your consent, contact that company. Questions about the CRM For Claims system itself: info@crmforclaims.com.
